- You have a disability, and
- You have a work history or have a parent or spouse who has a work history,
and
- You are over 18 and receive disability benefits through Social Security
Disability Insurance (SSDI) or Retirement, Survivors, and Disability
Insurance (RSDI) based on the work history, and
- You have received your SSDI or RSDI payments for 24 months.
Persons with end stage renal disease:
- You have permanent kidney failure, and
- You have a work history or have a parent or spouse who has a work history,
and
- You have completed three months of kidney dialysis, or
- You have received a kidney transplant, or
- You have been admitted to a hospital as an inpatient in preparation for an
immediate kidney transplant, or
- Your kidney transplant fails and you resume a regular course of kidney
dialysis.
Persons who are 65 years of age:
- You have reached 65 years of age, and
- You have a work history or have a spouse who has a work history.
